Introduction

Thank you for your interest in taking our survey on Whiplash!

The goal of this survey is to obtain stories from people who have experienced whiplash within the last three years. An important part of the Whiplash Prevention Campaign is to express the stories of those who have actually experienced whiplash. How did whiplash affect your work and family life? These are important factors for raising awareness about whiplash and whiplash prevention.

The Canadian Institute for the Relief of Pain and Disability is a registered Canadian charity dedicated to the prevention and reduction of pain, pain-related suffering and disability. We are working with the British Columbia Injury Research and Prevention Unit and Auto21 Networks of Centres of Excellence to end unnecessary suffering from whiplash by taking preventive action.

Your input will help us create a new whiplash prevention website.

Before participating in the survey, we require some basic personal information. This information will only be used to keep you updated about our survey results and the new Whiplash Prevention website. We value your participation in our activities and have a very strict privacy policy. CIRPD only uses your contact details for the stated purpose.
